If you are not able to contact your provider, or you received your shots at a mass-vaccination site, then you should do what the CDS recommends. That means contacting the immunization information system of your state’s health department (IIS).

Immunizations against SARS-CoV-2 must be reported to the state’s IIS by all vaccination providers. Each state’s IIS is included on the CDC’s website, including contact information.

Usually, it will take around two weeks to obtain a copy of the immunization record from the state health department. However, this period is not standard, and it can vary from a state to another. Some states will give you a second physical card, while other states can provide you with a digital record.
